Our organisation is looking for an experienced, dedicated and dynamic individual to undertake the role of Care Coordinator, coordinating care services for our new Supported Living and Domiciliary Care Services in Cornwall.
PURPOSE OF THE POSITION

To ensure the delivery of a high quality of service to clients by line managing care workers
-
To establish a caring environment for service users through high standards of professional practice and to promote dignity, respect, choice, and self esteem for each individual
-
To ensure that the requirements of the National Minimum Standards for Domiciliary Care agencies and that CQC regulations are adhered too.
PRINCIPAL RESPONSIBILITIES As a Regional Care Coordinator, your responsibilities will be as follows:

Ensure the service users receive care and support as agreed, and ensure that all calls are covered within your designated area

To assist in the planning and implementing of care packages for individual homecare service users

To manage and delegate the allocation of support workers to specific service users in line with their individual needs and the carer’s experience/expertise

Ensure homecare workers’ availability and capacity is fulfilled

To ensure that all the Community Support Workers who work with new referrals are fully briefed to enable them to carry out their role effectively
-
To participate in staff training/development when this is deemed appropriate.

To carry out on-call duties
PERSON SPECIFICATION
-
Minimum ONE YEAR’S EXPERIENCE as a Senior Care worker, Team Leader or Shift Lead for domiciliary care or supported living services, coordinating, scheduling and supervising staff to meet clients needs (preferred)

Understanding of principles of good care and CQC regulations

Committed to quality care and services provision

Good IT skills, including MS Outook, Microsoft Excel
- Familiar with a Care Management System , preferably Access Care Planner and People Planner

Flexible, willing to work outside normal office hours when required

Good team player
-
Self-Motivated with the ability to work under own initiative and prioritise workload
-
Excellent communication skills with the ability to built and maintain god relations with staff and clients alike

Driver with a full UK Driving Licence
